TN Immunization Requirements from TN Department of Health (Rule Chapter 1200-14-1-.29)

All children entering 7th grade are required to have:

• Tentanus-diphtheria-pertussis booster (Tdap)

• Verification of immunity to varicella (2 doses or history of disease)
